Prevention and Screenings

Having the right information and screening done can save lives.

Employees, are you healthy?  Do you know your numbers?

Call and set-up an appointment with your physician annually.  No medical information is shared.  The form just needs signed off by your physician, verifying your physical has been done.  One call may save your life.  Do it for your loved ones.  Prevention and early treatment can prevent catastrophic illnesses.  It is important to remember to tell the person scheduling the appointment that you want an annual preventive exam to prevent incurring costs associated with any diagnostic tests. A preventative schedule is attached for guidance of preventative care allowances.   Please return the completed form to the AIS HR office within one week of your visit.
